
If you are interested to attend intensive exercise program at our facility, you must follow the application process listed below:
1. Completely fill out
and submit back to us the Member Health Information Form. Please be as
thorough as possible, so
that we have a better understanding of
your child's needs. This form can be mailed to you upon request or you
can
get it off our web site:

2. Upon approval for our program, you will need
to submit a written report regarding recent hip x-rays (within the last
6 months) specifically an A/P view of your
child's hips to rule out hip subluxation or dislocation. This is only
required
for those children using the TheraSuit.
3. Once we have received the above
documentation, our office will contact you to schedule a time for your
child to
attend our center.
4. After your child has been scheduled to
attend, we will send you a package with information regarding
hotels/accommodations, shopping, movie
theaters, restaurants, things to do in the area & a map to help
assist you
with your needs during your visit.
If you are within driving distance of our center; this package
will not be mailed to
you.
5. In order to
secure your reservation at our center, we must receive a deposit in the
amount of $1,000.00 sixty days
prior to your child's scheduled session &
the remainder of the balance is due when your child attends. This
deposit
is refundable if cancellation is due to
medical reasons. If a deposit is not received to secure your
reservation, your
spot will become available to someone else.
